About The Position

TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. Founded in 1976, TRU affirms life at every step of the journey with illness and loss, providing a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. The PACE Clinic Scheduler/Care Coordinator is responsible for scheduling all appointments with specialists and medical clinic appointments. This role coordinates and maintains appointment data sheets to ensure CMS regulations are followed, guaranteeing participants receive specified care and appointments. The coordinator collaborates with outside providers to establish working relationships for participant care and works with the PACE Medical Records office to ensure timely receipt of provider notes. Participation in clinic meetings and overall clinic planning is also expected.

Requirements

  • 5 years administrative experience in a medical-related field
  • Computer proficient and able to work with various databases
  • Competent with Word, Excel and PowerPoint programs
  • Ability to collaborate with team and manage several projects simultaneously

Responsibilities

  • Scheduling all appointments for participants with specialists and outside medical appointments when requested by the medical team.
  • Creating authorizations for scheduled appointments within the EMR.
  • Documenting in the medical record as appropriate to scope of practice.
  • Performing any necessary filing or clerical duties necessary to complete charting requirements.
  • Assisting in prioritizing the scheduling of participants.
  • Collaborating with outside providers to establish a working relationship to provide appointments and care for participants.
  • Collaborating with PACE Medical Records office to confirm that provider notes are being received in a timely manner.
  • Participating in clinic meetings and overall planning related to the clinic.
