About The Position

North Texas Behavioral Health Authority (NTBHA) is seeking a passionate Care Coordinator for Rockwall County to join their team. NTBHA is dedicated to creating a collaborative system of care for individuals with severe mental illness, serious emotional disturbances, and substance use disorders. This role focuses on providing timely, comprehensive, trauma-informed, person-centered, and recovery-oriented interventions to help individuals in crisis achieve stability in the community. The Care Coordinator will serve Rockwall County, working closely with the NTBHA Utilization Management Department and community stakeholders to ensure continuity of care and crisis response. This position aims to provide alternatives to arrest, emergency department visits, and inpatient psychiatric care by offering screenings, assessments, crisis follow-up, and transitional services. The goal is to connect individuals with mental illness and/or co-occurring disorders to services, prevent crises, and eliminate barriers to treatment. The Care Coordinator will actively participate in staffing, collaborate with internal departments, law enforcement, hospitals, and community agencies, and provide skills training and case management to reduce recidivism, hospitalization, and homelessness. The role requires flexibility in scheduling to meet organizational needs, including potential evening and weekend work, and may involve significant travel within the NTBHA catchment area.

Requirements

  • Bachelor’s degree in psychology, social work, or a related behavioral health field.
  • One or more years of experience with direct care of persons with serious mental illness and/or substance use disorders.
  • Experience participating as a member of a treatment team that develops and monitors treatment plans for persons with chronic & serious mental illness.
  • Current knowledge of local community resources, providers, and social support for homeless and indigent populations within the NTBHA service area.
  • Must be able to work on and have knowledge of a PC (personal computer).
  • Advanced Microsoft Office Suite skills - Outlook, Word, Excel, PowerPoint, etc., with an ability to become familiar with company-specific programs and software.
  • Possess and maintain a valid driver’s license.
  • Pass and maintain a clear background record as required for healthcare organization under state and federal contracts.

Nice To Haves

  • A master’s degree is preferred.
  • Preference given to experience in psychiatric crisis service settings.
  • Understanding of and experience with HHSC programming and systems preferred, including Texas Resilience and Recovery, CMBHS, and CARE.

Responsibilities

  • Provide timely intervention in the least restrictive level of care.
  • Deliver comprehensive, trauma-informed, person-centered, recovery and resilience-oriented efforts.
  • Provide continuity of care and crisis response solutions through intersystem and interdepartmental functions.
  • Perform complex services including screenings and assessments, crisis follow-up, and transitional services.
  • Connect individuals with mental illness and/or a co-occurring disorder to other services.
  • Eliminate barriers to treatment.
  • Actively participate in staffing's to determine best treatment options.
  • Collaborate and work effectively with internal departments, law enforcement, state and local hospitals, and community agencies and organizations.
  • Provide skills training and case management targeting identified individual risks and needs relative to reducing recidivism, hospitalization and/or homelessness.
  • Create, analyze, prepare, and maintain records, files, documents, reports, and correspondence.
  • Recognize problems and identify and facilitate solutions.
  • Communicate effectively and use effective interpersonal skills to communicate ideas and complex concepts with individuals from diverse cultural and socio-economic backgrounds through oral and written communication.
  • Constructively and efficiently work with other professionals and staff, community representatives, persons with disabilities, and family members.
  • Foster strong, positive, and effective working relationships with inter-agency and intra-agency team members, encouraging and supporting interaction among various team members across organizational lines.
