Requirements

  • Bachelor’s degree in Social Work or related behavioral health field OR Associates degree with at least 2 (two) years of experience in an acute or transitional outpatient setting
  • All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment

Nice To Haves

  • Master’s degree in behavioral health or related field
  • Bilingual (Spanish) preferred

Responsibilities

  • Facilitates admission intakes, treatment planning, discharge planning, and personalized coordination of care for individuals who are in crisis and/or stabilizing from crisis.
  • Works collaboratively with the Interdisciplinary treatment team to coordinate care and discharge planning for patients.
  • Responsible for a caseload that may require frequent communication.
  • Acts as an ambassador to community partners.
  • Conducts a thorough, accurate, clinically sound Psychosocial assessment, diagnosis, and LOCUS score with the individual in a timely manner.
  • Obtains signatures from licensed therapist as assigned.
  • Educates and informs patients of consents for treatment, communication, and compliance.
  • Gathers collateral information from the individual, their clinic, other providers, etc. to provide a more thorough assessment of what the individual's immediate needs are for treatment and discharge.
  • Collaborates with the Treatment Team to develop and implement an individualized person-centered treatment plan that is appropriate for a crisis and/or a transitions-of-care intervention.
  • Reviews and revises the signed Treatment Plan with each patient and any other involved parties.
  • Actively coordinates care for individuals to ensure services are coordinated with facility staff.
  • Identifies and addresses gaps in service needs for participating parties in service delivery to the individual and makes appropriate recommendations to meet individuals’ needs upon discharge.
  • Collaborates with Interdisciplinary treatment team to engage, monitor, and communicate with the individual.
  • Facilitates and leads regular case planning with the involved Provider as new information is gathered to ensure the most clinically appropriate individual disposition.
  • Documents all services and individual activities in the medical record regarding assessment, interventions, coordination of care, treatment planning, and discharge planning.
  • Maintains a working knowledge of the Contracted Health Plan system and potential funding.
  • Participates in individual and group supervision as required.
  • Participates in ongoing education, including in-services, training, and other activities to maintain and improve competency.
  • Demonstrates competency through post-testing, skill observation, and performance as assessed by direct supervisor.
  • Follows up with Director about all cases weekly.
  • Performs all other duties as assigned.
