About The Position

Monday – Friday (ability to participate in on call rotations) Day Shift $22.06 - $30.10 DOE Pierce County’s first Certified Community Behavioral Health Clinic; Comprehensive life Resources is looking for a Care Coordinator to join our WISe team in Downtown Tacoma, Washington. Scope A formal member of the WISe team who is specially trained to coordinate and facilitate the WISe process for an individual youth and family and provide advanced care coordination activities within the phases and activities of WISe. The Care Coordinator will participate in the rotation of the off-hours crisis phone to support clients (compensation provided) and provide crisis support to youth and families. This position requires driving to and transporting clients based on program needs.

Requirements

  • AA degree in a human services field with two years relevant experience or BA required.
  • WA State credential or ability to become credentialed
  • Must have a valid driver’s license, proof of insurance, and a working vehicle.
  • Proficiency in common Microsoft applications
  • Strong written and verbal communication skills
  • Demonstrated ability to prioritize deadlines, work independently, take initiative, and maintain confidentiality

Responsibilities

  • Provide both case management and behavioral health support services, including, but not limited to assisting clients with paperwork, self-care, advocacy, and resource acquisition.
  • Be trained and use Motivational Interviewing and other evidence-based practices
  • Provide psychoeducation to clients and their natural support about behavioral health conditions, treatments and navigating systems.
  • Provide case coordination, working within a multi-disciplinary team environment.
  • Facilitate psychoeducation and skills groups as defined by program needs.
  • Connect clients with internal and external support and services.
  • Attend/facilitate treatment team meetings, staff meetings, and designated staff training and consultation sessions as assigned.
  • Ensure that ethical and professional standards of practice are maintained by following all CLR policies and seeking regular consultation.
  • Actively participate in clinical supervision and training for continued professional growth.
