About The Position

The Veterans Care Coordinator/Navigator will provide care coordination and patient navigation services to veterans in Jefferson County who are engaged in behavioral health services at the Jefferson County Community Mental Health Program (CMHP) – CCBHC/Certified Community Behavioral Health Clinic. This role involves direct interaction with veterans, telephone support, transportation assistance, and connecting veterans with medical and community services. The coordinator will also be responsible for case management, facilitating access to recovery activities, and collaborating with a multidisciplinary treatment team, including clinicians, medical practitioners, and peer support. A key aspect of the role includes working with the CCBHC-CMHP Prevention-Health Education Team on veterans' suicide prevention strategies and community trainings. The position also entails modeling BestCare's mission and values, supporting the organization's commitment to equity and inclusion, and maintaining professionalism and compliance with relevant regulations.

Requirements

High School diploma or equivalent; bachelor’s degree or 5 years of relevant experience in the behavioral health field.
Familiarity with veterans/services as well as mental illness and substance abusing populations.
If in recovery from mental illness and/or substance abuse, must be able to demonstrate continuous sobriety under non-residential, independent living conditions for the immediate past two years.
CADC-I or QMHA-I certification required or eligibility to register within 1 week of hiring to obtain QMHA-I within 6 months or CADC-I within 1 year.
Must maintain a valid Oregon Driver License or ability to obtain one upon hire and be insurable under the organization’s auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations).
Must have a reliable vehicle with adequate insurance to travel as needed.

Responsibilities

Direct interactions with veterans who are open in behavioral services at the Jefferson County CMHP-CCBHC.
Telephone contacts with veterans in CMHP-CCBHC services for support, crisis mental health services, and rapid access to SUD assessment, detox, and treatment services.
Reminder calls to veterans scheduled for assessments, groups, individual sessions, medication management appointments, and other BestCare services.
Transportation of clients to counseling/treatment appointments, when appropriate and approved by supervisor.
Helping veterans connect with medical services, including primary care, and OHP Assisters.
Referrals to and assistance with accessing various community services (e.g., Food Bank, LINC, primary care/medical services, community college, parenting).
Case management services, including documentation.
Helping clients develop and connect to community-based pro-social or wellness-oriented recovery activities and veterans’ activities.
Being part of a CCBHC-CMHP behavioral health treatment team and working closely with clinicians, medical practitioners, and peer support.
Connecting with the CCBHC-CMHP Prevention-Health Education Team to work on veterans’ suicide prevention strategies, initiatives, and community trainings.
Other duties as assigned by the Program Director or Mental Health-Adult Team Program Manager.
