About The Position

At Mind&Mobility, the mission is to help seniors live independently, confidently, and joyfully through compassionate in-home care, personalized therapy, and innovative brain health programs. The Care Coordinator is responsible for driving patient growth for the outpatient therapy clinic by developing referral relationships, coordinating new patient admissions, and ensuring a seamless transition from referral to therapy services. This role combines sales, relationship management, and care coordination, working closely with physicians, referral sources, and the clinical team to increase new patient evaluations and ensure patients receive the therapy services they need.

  • Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse

Responsibilities

  • Develop and maintain relationships with physicians, medical offices, hospitals, and community referral sources
  • Generate new patient referrals and support the growth of the clinic's therapy services
  • Coordinate new patient admissions from referral through initial evaluation
  • Serve as a liaison between referral sources, patients, and the therapy team
  • Educate referral partners about therapy services and patient benefits
  • Track and manage referral pipelines and follow up on potential patient opportunities
  • Collaborate with clinic leadership and therapists to ensure a positive patient experience
  • Monitor patient progress to support continuity of care and maximize therapy outcomes
  • Represent the clinic at community events, networking opportunities, and professional meetings when appropriate
