Requirements

• Must possess a high school diploma or GED, Bachelors degree preferred
• Must be able to work in an outcomes and deadline oriented environment.
• Must be able to cope effectively with interruptions
• Must use time effectively and have excellent “People” skills

Responsibilities

• Ensuring appropriate communication with primary care providers for targeted patients across different care settings (Hospital, Emergency Department, Clinics, and Primary Care Offices).
• Working directly with patients over the phone, and in person, depending on the needs of the individual patient and the program.
