About The Position

The Care Coordinator Social Worker at Lankenau Medical Center is responsible for exemplary cross-continuum care coordination, including psychosocial assessments and interventions. The role works with physicians, peers, community colleagues, and others to coordinate care across the health care continuum, identify opportunities to improve patient care and services, improve population health, and support collaborative practices that reflect Main Line Health System’s commitment to patient-centered care and community engagement.

The position provides coverage in the Emergency Department on a per diem basis, with varied shifts and 12-hour shifts. Main Line Health offers access to education and development classes, ongoing feedback, and career development through performance appraisals and one-to-one communication with the manager.

Requirements

• At least 5 years of recent clinical experience in a physician’s office, clinic, hospital, emergency services (EMS) setting, or equivalent experience.
• Master’s Degree in Social Work from a CSWE-accredited program.

Nice To Haves

• Leadership experience.
• Licensed Social Worker; strongly preferred.
