About The Position

The Care Coordinator uses a team-based approach focused on a continuous partnership between the patient, family, caregiver, physician, clinicians, hospital and community resources to provide coordinated care for complex patients. The Care Coordinator is responsible for direct and indirect patient care across the care continuum, with an emphasis on assisting high-risk patients in the community, including Medicare, Medicaid, VHA and other payor mix beneficiaries. Activities will promote, maintain and improve the health of patients by creating and maintaining a centralized, multidisciplinary care plan and will provide social support, advocate for individuals, address community health needs and perform community outreach such as health screenings.

Requirements

  • At least two years of experience in a clinical setting as a Registered Nurse, Advanced Practice Provider (NP/PA), Clinical Social Worker, or similar.
  • Unrestricted license to practice as a Registered Nurse (RN) in the State of Colorado, or ability to obtain prior to start date.
  • Basic Life Support (BLS). May be obtained upon hire.

Nice To Haves

  • Bachelor's (BSN) or Master's (MSN) in Nursing preferred.
  • Care coordination or case management experience preferred.
  • Experience with EPIC preferred.
  • Bilingual in Spanish preferred.
  • Equivalent combinations of experience and education may be considered.

Responsibilities

  • Provide coordinated care for complex patients using a team-based approach.
