About The Position

Purpose of Position:

Coordinate the overall interdisciplinary plan of care for a patient, serving as the liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. May also provide leadership to direct reports or cross-functional team.

Requirements

• Must have a specialized knowledge of the particular patient population
• Knowledge of chart documentation
• Knowledge of referral services
• Above average nursing skills
• Ability to manage heavy case loads
• Ability to communicate with and serve patients in very emotional circumstances
• Associate’s Degree in Nursing (Bachelor’s preferred)
• 5 year(s) working as a Registered Nurse (8 years preferred)
• 1 year supervisory experience
• Alaska Registered Nursing License, Basic Life Support, and ACLS within one year of hire

Responsibilities

• Promote patients' independence by establishing patient care goals and teaching patients and families to understand conditions, medications, and self-care skills
• Network with providers and agencies to provide ongoing follow-up and care management services, monitoring annual and 6 month assessments of client service plans and providing a quarterly reports of care management statistics to management
• Maintain contact with clients and providers (when required) to verify the amount, scope and duration of service identified on the plan of care at least once a month, updating the client’s current condition and progress
• Plan and organize specialty clinics and other events in support of healthcare promotion
• Act as a relief RN in the Outpatient Clinic when necessary
• Perform other duties as assigned
