The licensed RN care coordinator demonstrates expertise in a healthcare setting in the assessment and treatment of patients along the continuum of care. This role involves developing and implementing discharge planning within an interdisciplinary healthcare team, and addressing complex clinical care needs by identifying and removing barriers that prevent optimum access to needed post-acute care. The care coordinator works collaboratively with providers to identify discharge barriers and delays to optimize the efficient use of acute care hospital days and prevent prolonged length of stay. They provide clinical leadership to their healthcare teams and ensure daily goals are met according to the patient’s plan of care.
