About The Position

As a Care Coordinator Nurse at Riddle Hospital, you will be the voice and guide for orchestrating your patients’ care across the healthcare continuum. You will work with physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care experiences and services, improve population health, reduce cost, and achieve collaborative practices that exemplify Main Line Health System’s commitment to patient centered care, community engagement, and population health. You will use your excellent communication and advocacy skills to partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive plan of care which includes health promotion, disease prevention, and disease management activities. Main Line Health offers opportunities for nurses to pursue certifications or advanced nursing degrees through its Tuition Reimbursement Program, providing up to $6,000 per year. Nurses are also encouraged to become engaged by joining hospital & system-wide Nursing Committees, Unit Councils, and evidence-based research projects. Requirements

  • Three years of recent clinical experience required.
  • Active PA RN license required.
  • Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire

Nice To Haves

  • Leadership experience preferred.
  • Master’s Degree preferred.
