About The Position

Hamilton Health Center is seeking an LPN Care Coordinator to support patient-centered care at our Newport Family Care location. This role promotes collaboration among the patient-centered care team to ensure quality care, increase preventive care utilization, and reduce emergency room visits and hospital readmissions. The LPN Care Coordinator provides care coordination and case management for patients, including individuals with higher-risk or complex care needs, while helping patients and families navigate healthcare services and treatment plans. The position works collaboratively with providers, Medical Assistants, Patient Navigators, Pharmacy, and other members of the care team to support continuity of care and positive patient outcomes.

Requirements

  • High school diploma or GED
  • Graduate of an accredited Licensed Practical Nursing program
  • Current license to practice as a Licensed Practical Nurse in the Commonwealth of Pennsylvania
  • At least two (2) years of experience working with patients and families in a community setting, public health, outreach, social work, or a related field, with progressively responsible experience
  • Prior experience with care coordination or care management in a clinical setting
  • Proficiency in basic data entry and computer skills
  • Strong interpersonal and communication skills
  • Ability to work collaboratively within an interdisciplinary care team

Responsibilities

  • Provide care coordination services involving case management, client advocacy, and goal-oriented action plans for patients
  • Co-manage higher-risk patients through assessment and care coordination activities
  • Maintain ongoing interaction with patients and families to support adherence to treatment plans, self-management, and shared decision-making
  • Educate patients, families, and caregivers regarding the Patient-Centered Care Model and applicable care needs
  • Serve as a liaison for patients regarding medical and dental benefits
  • Coordinate with providers and members of the patient-centered care team to support continuity and quality of care
  • Monitor care delivery by documenting patient progress, identifying barriers to care, and maintaining communication within the care team
  • Conduct home and hospital visits when required as part of care coordination responsibilities
  • Ensure compliance with HIPAA requirements and maintain the confidentiality of protected information
  • Collect and utilize data to support service improvement activities and prepare required monthly reports
  • Perform additional job-related duties as assigned
