About The Position

The Care Coordinator II supports care management activities and assigned teams to help ensure members receive services from healthcare providers and partners, with continuity of care and member satisfaction. The role engages members through telephone outreach and home visits, and documents plans for care, services, and related activities. This remote position serves Missouri foster care youth ages 5–15 with behavioral health needs. The schedule is Monday through Friday, 8 a.m. to 5 p.m.

Requirements

High school diploma or GED.
1–2 years of related experience.

Responsibilities

Contact members by phone or home visit to discuss care or service plans, next steps, resources, questions or concerns about recommended care, and ongoing education as appropriate.
Coordinate care activities according to care or service plans, working with healthcare and community providers, partners, members, and caregivers to accommodate changes or progress.
Support member and provider inquiries, requests, and concerns related to care or service plans.
Communicate with care managers, practitioners, and others to facilitate member services and continuity of care.
Support service assessments or screenings and document members’ care needs.
Document and maintain member records in accordance with state and regulatory requirements and distribute records to providers as needed.
Follow standards of practice and policies that comply with contractual requirements and regulatory guidelines and standards.
Identify needs and make referrals to care managers, community-based organizations, and disease managers.
Educate members about available benefits and resources.
Perform other duties as assigned and comply with all policies and standards.
