Assists all patients through the Firelands Health by acting as a patient advocate; coordinates continuity of patient care with internal providers and external healthcare organizations and facilities; and manages high risk patient care. The Care Coordinator II will assist all patients through the Firelands Health by acting as a patient advocate and navigator. They will participate in Care Management team meetings and quality improvement initiatives, support patient self-management of disease and behavior modification interventions, and coordinate continuity of patient care with external healthcare organizations and facilities. This role also involves coordinating continuity of patient care with patients and families following hospital admission, discharge, and ER visits, as well as coordinating care for patient populations with complex health needs focusing on improving outcomes, reducing risk, and supporting appropriate utilization of healthcare services. The position includes conducting comprehensive, preventive screenings for patients and/or assisting all support staff in daily patient interactions as needed. It also promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans, facilitating patient medication management based upon standing orders and protocols, and participating on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Medical Home initiatives. The role involves evaluating clinical care, utilization of resources, and development of new clinical tools, forms, and procedures. Additionally, it includes labeling, documenting, and tracking all specimens or tests ordered/received following set standards and reporting all test results to the physician. Charting care coordination on the day of visit to ensure a documented, organized, and chronological medical record with the submission of timely, accurate, and complete information to the Central Billing Office is also a key responsibility. The position promotes wellness by providing patient education materials and communicating the physicians’ advice and instructions to patients/families, and assists with scheduling appointments, tests, or procedures.
