About The Position

The Care Coordinator I plays a vital role in connecting patients with the healthcare services and community resources they need to achieve better health outcomes. This position coordinates referrals across NMHSI sites, collaborates with providers and community partners, and helps patients navigate the healthcare system by removing barriers to care. Working closely with clinical teams, Community Health Workers, and external organizations, the Care Coordinator supports patients with moderate to complex healthcare needs while promoting quality, continuity, and efficiency in care delivery.

Requirements

  • Bachelor of Science or Associate's degree in a clinical science or related healthcare field OR a minimum of two (2) years of experience in a related field
  • Two (2) or more years of ambulatory care experience
  • Experience working with patients managing chronic illnesses
  • Experience with data compilation and analysis
  • Knowledge of medical terminology
  • Experience with quality improvement initiatives and process improvement
  • Strong organizational and time management skills with the ability to manage multiple priorities
  • Intermediate computer skills and proficiency with healthcare technology systems
  • Ability to work independently while maintaining excellent attention to detail
  • Strong communication, problem-solving, and interpersonal skills
  • Current Basic Life Support (BLS)/CPR certification or ability to obtain, if required by licensure or certification
  • Ability to work collaboratively with providers, care teams, patients, families, and community partners
  • Commitment to providing compassionate, patient-centered care and improving access to healthcare services

Nice To Haves

  • Experience in a community health or Federally Qualified Health Center (FQHC) setting preferred

Responsibilities

  • Coordinate referral activities across all NMHSI locations to ensure timely, high-quality patient care
  • Process and monitor referrals for medical, behavioral health, specialty care, and community services
  • Train staff on referral workflows and best practices for processing and following referrals
  • Coordinate referrals to the Community Health Worker (CHW) program and collaborate with local health departments to reduce barriers to care
  • Identify and connect patients with appropriate community resources and support services
  • Assist patients and families in understanding healthcare options and navigating the referral process
  • Create, maintain, and monitor referral databases to ensure accurate documentation and follow-up
  • Collaborate with providers, care teams, and community partners to improve care coordination and patient outcomes
  • Perform other duties as assigned
