Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Waco, Texas area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support. This Care Coordinator works directly within partnered facilities alongside providers, patients, families, and facility staff to support continuity of care, with a primary focus on Remote Patient Monitoring (RPM). Responsibilities include supporting RPM enrollment and compliance, coordinating follow-up care, addressing barriers to care, communicating patient needs, and maintaining accurate and timely documentation within the EMR. This role is very family-practice focused, highly relationship-driven, and centered around being proactive versus reactive when it comes to patient care. Qualified candidates must possess a minimum of a Medical Assistant (MA) certification with at least one-year of clinical experience, and be compassionate, detail-oriented professionals who thrive in a fast-paced environment. Candidates should demonstrate strong organizational skills, adaptability, and clear, professional communication.
