About The Position

Lighthouse Behavioral Wellness Centers is seeking a Care Coordinator to guide individuals through mental illness, trauma, or addiction. This role focuses on supporting those who have recently experienced crisis or hospitalization, helping them navigate multiple systems, engage with new treatment teams, and adjust to changes outside of the clinical setting. The Care Coordinator will work within a child-centered system of care, prioritizing the needs of the child and family to facilitate seamless engagement into outpatient services and aftercare, preventing out-of-home or out-of-school placements. The position emphasizes working with consumers and families in their homes, schools, and communities, incorporating social supports and focusing on strengths, preferences, and culture. This role is supervised by the Director of Outpatient Services.

Requirements

  • A Bachelor or Master Degree in Education, Psychology, Social Work, Sociology, Criminal Justice or similar behavioral health field approved by ODMHSAS; OR A current license as a registered nurse in Oklahoma with behavioral health experience; OR A Bachelor’s or Master’s degree in any field with 6 months of behavioral health experience; OR 60 college credit hours in any field and 12 months of behavioral health experience; OR A high school diploma or equivalent and a total of 36 months of direct, documented experience working with persons who live with mental illness and/or substance use issues.
  • Applicants must be able to provide documented proof from previous employers to justify behavioral health experience.
  • Behavioral Health Case Management Certification and Behavioral Health Wellness Coach Certification is required within the first three months of employment and must be kept current annually.
  • Current certification as a Behavioral Health Case Manager II as described in ODMHSAS Standards ad Criteria for Certified Behavioral Health Case Managers 450:50-3-1.
  • Current certification as a Behavioral Health Wellness Coach as described in ODMHSAS Standards ad Criteria for Certified Behavioral Health Case Managers 450:50-5-145.
  • An expectation of core competencies in relation to children and families, as well as individuals with co-occurring disorders, trauma informed care and collaborative care.
  • Knowledge of the principles and practices of social work and/or psychology; and of current social and economic problems, principles of family economics and budgeting, human relations, community, state and national health and welfare resources, social assistance requirements, and psychological terminology.
  • Knowledge of transitional and/or aftercare needed after a crisis or hospitalization and the unique situations that may arise as consumers attempt to engage in outpatient services.
  • Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.
  • Knowledge of the elements of personal wellness and how to collaborate with others to identify their personal strengths and goals within the Eight Dimensions of Wellness.
  • Able to establish working relationships within an integrated team to coordinate and facilitate care by assessing consumer needs, evaluating treatment options, creating care plans, coordinating care with internal and external providers, and monitoring progress.
  • Ability to work effectively and courteously with children and families from various cultures and backgrounds, and to exercise good clinical judgment.
  • Must possess skill in working cooperatively and effectively with clients, staff, management and other professional and community groups; in exercising mature judgment in dealing with people; in presenting ideas clearly and accurately; in reading and comprehending the English language; and in communicating effectively, both orally and in writing.

Nice To Haves

  • One year of work experience with juveniles in the mental health or social service field preferred.
  • Specialized training and/or certification in mental health services or related field as may be required in order to provide adjunctive services, ie. Systems of Care, Wraparound training.
  • Ability to commute/relocate: Reliably commute or planning to relocate before starting work (Preferred)

Responsibilities

  • Provide individualized case management, rehabilitative, and wellness services in various settings to individuals, families, and groups to promote emotional or psychological change, alleviate mental disorders, facilitate recovery, and encourage engagement in the treatment setting.
  • Schedule, coordinate, and facilitate wraparound family team meetings, ensuring formal and informal supports are included.
  • Complete Strengths Needs Cultural Diversity (SNCD) Assessments, Care Plans, Functional Assessments, Crisis Plans, and Safety Plans.
  • Complete OKSOC Assessments and other specified documentation as assigned.
  • Enter data into the YIS system according to ODMHSAS requirements and timelines.
  • Provide crisis management and stabilization as assigned.
  • Monitor and evaluate the needs and abilities of the consumer, assisting them to function more adequately through their own strengths and community resources.
  • Recognize individual client and family barriers to social, economic, and educational opportunities and assist in overcoming them.
  • Assist clients to adjust and adapt themselves to their environment, care, and treatment.
  • Identify barriers to treatment and assist consumers in problem-solving and engaging in outpatient care to prevent regression and re-hospitalization.
  • Complete all agency documentation of services thoroughly, accurately, and in a timely manner.
  • Serve as a role model for wellness behaviors.
  • Apply the principles and processes of coaching when collaborating with others.
  • Provide referrals to community resources for housing, employment, medical, nutrition education, weight management, Oklahoma Tobacco Helpline, and other community or wellness-related services.
  • Develop cooperative working relationships within the local community to enhance consumer access to resources that support individualized Care Plan goals.
  • Perform all job-related travel normally associated with this position.
  • Meet agency standards of productivity and fiscal responsibility.
  • Prepare for and actively participate in huddles, Care Planning, and staff meetings.
  • Meet "satisfactory" expectations at any scheduled job performance evaluation, coaching, and/or Performance Improvement Plans.
  • Complete Lighthouse required training and resulting follow-up and consultation as required at hire, annually, and as directed by Supervisor.
  • Perform other reasonably related duties as assigned by the immediate supervisor or other management.
