Requirements

  • Registration or Certification with a state approved AOD certifying body. Must be registered or certified upon hire.
  • Passion and desire to work with individuals with a substance use disorder, using a compassionate and person-centered approach.
  • One year of experience in social services or related field preferred.
  • Strong oral and written communication skills.
  • Professional manner and appearance.
  • Ability to work independently and within a team.
  • Ability to take direction.
  • Ability to work effectively with diverse groups of clients, staff, and community members.
  • Organized, detail-oriented, and a creative problem-solver.
  • Proficient in computer skills and use of Microsoft products such as Outlook, Word, and Excel.
  • Must have a valid CA driver’s license.

Responsibilities

  • Registration or Certification with a state approved AOD certifying body. Must be registered or certified upon hire.
  • In collaboration with the multi-disciplinary treatment team, the Care Coordinator will assess participants to address any problems, needs, or barriers identified in the areas of: Housing, Employment, Education/Training, Financial, Legal, Sobriety/Social Support, SUD Treatment, Family/Social, and Medical/Psychiatric.
  • Conduct in-home, in-field and on-site assessments and care coordination/linkages
  • Assist the primary counselor with discharge-planning efforts, commencing at the time of admission on all assigned participants, including housing, employment, medical/psychiatric, financial, aftercare, etc.
  • Provide participants with high-quality referrals and linkages for necessary resources and services as identified in the Treatment Plan/Problem List, playing an active role in reducing barriers and ensuring participants have ‘actual’ access to needed services.
  • Act as a bridge between health and human service providers to ensure that information is appropriately exchanged, and participants are successfully linked to needed resources/services.
  • Coordinate care with other community agencies by assisting participants with eligibility, documentation, and applications, etc. needed to access services/resources.
  • Assist participants with transitions within the SUD system of care, referring to higher or lower level of care as appropriate.
  • Follow up with participants to ensure all scheduled appointments and linkages are completed.
  • Collaborate and communicate fully with other team members regarding participant behavior and provides weekly updates of participant’s progress.
  • Routinely identify new and existing community resources that participants may benefit from.
  • Conduct outreach to the community to develop relationships with other providers and organizations that can benefit program participants in supporting their recovery.
  • Effectively and professionally communicate with participants, staff, and other service providers through telephone, emails, letters, and progress notes and or reports to the County, State, and other service providers on behalf of the participant.
  • Provide individual counseling, group facilitation, and crisis intervention as part of a multi-disciplinary team, as needed.
  • Provide regular, comprehensive progress note charting in participant’s electronic health record, which documents contacts, progress towards identified goals, program participation, referrals, discharge planning, and other care coordination activities.
  • Maintain accurate and timely documentation of care coordination or other services provided.
  • Develop and submit accurate reports regarding services delivered, clients served, outcome of referrals, retention rates, and other information as needed.
  • Exhibit full knowledge and support of the philosophy and objectives of CLARE|MATRIX.
  • Professionally conduct self in areas of dress, language, attitude, and behavior.
  • Fully understands and maintains policies regarding professional ethics.
  • Attend in-service training and relevant educational workshops.
  • Attend all required staff meetings.
  • Additional tasks, projects, and responsibilities as assigned by the supervisor or designee
