About The Position
Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System’s implementation of population health initiatives. Population health initiatives focus on preventive care and aim to enhance the patient experience and health status, improve the health of patient populations, and reduce utilization and healthcare costs. MSHP provides the infrastructure to support these initiatives, including services such as care coordination, information technology, workflow optimization, physician engagement, and quality reporting. The Department of Social Work Services plays a leadership role in MSHP Care Management, which offers essential short-term and long-term care management services for a diverse group of individuals with various insurances who are part of value-based contracts. The primary goal of MSHP Care Management is to deliver standardized care management services that address patients’ medical and psychosocial needs, thereby improving quality and reducing unnecessary healthcare costs.
