About The Position
Supports patients across the continuum of care by coordinating transitions of care, conducting patient outreach, supporting quality and care gap closure, and facilitating effective care coordination within the ambulatory setting between patients, providers, and post‑acute settings. This role functions within the multidisciplinary Care Coordination team established to improve patient experience, clinical outcomes, and performance on quality and population health initiatives.
Requirements
- High school diploma or equivalent required.
- One year of healthcare/medical (hospital or clinic/office) experience with exposure to medical terminology is required.
Nice To Haves
- Prior experience in transitions of care, care coordination, outreach, ambulatory care, population health, and/or quality program support preferred.
