About The Position
The Social Worker, in collaboration with the clinical team, provides patient and family advocacy, discharge planning, and psychosocial intervention. The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources. When indicated, the Social Worker will assist with crisis intervention and legal reporting requirements.
Requirements
- Graduation from an accredited School of Social Work
- Master’s degree in Social Work
- Current LMSW
- Basic computer and keyboarding skills
- Three years social work experience
Nice To Haves
- CCM Certified
- Meditech Electronic Medical Record experience
- Experience in the hospital setting
