About The Position

New Freedom’s Care Coordination Program Manager plays a crucial role in delivering exceptional care coordination services to our members within our intensive outpatient and supportive housing facilities. In this position, the Care Coordination Program Manager assumes responsibility for coordinating all clinic, wellness, and laboratory operations onsite, aligning them with regulatory and compliance standards as well as with our company mission and values. Additionally, they actively collaborate with services to ensure our members have access to the best resources available. Beyond these responsibilities, the Care Coordination Program Manager serves as the manager of our internal Care Coordination department. They provide guidance and support, going above and beyond to surpass the continuum of care needs of our members. By excelling in this multifaceted role, our Care Coordination Program Manager ensures that New Freedom's commitment to delivering exceptional member care and experiences remains unwavering.

Responsibilities

  • Serve as primary liaison with third‑party medical, MAT, lab, and wellness providers to support member outcomes.
  • Coordinate care with external partners and internal departments to ensure seamless service integration.
  • Provide direct oversight to Care Coordination Case Management staff and promote health and wellness practices.
  • Manage resources and workflows across care coordination services within New Freedom facilities.
  • Develop, track, and update reports, metrics, and systems that support care coordination operations.
  • Support staff with problem‑solving, conflict resolution, coaching, and performance development in partnership with leadership and HR.
  • Ensure accuracy and compliance in EMR documentation, regulatory requirements, policies, and procedures.
  • Contribute to quarterly goal‑setting, maintain accountability toward KPIs, and participate in required supervision and continuing education.
  • Performing additional duties as assigned to support our mission.
