About The Position

Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low-income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The HIV Care Coordination Program (CCR) addresses HIV healthcare disparities by facilitating access to care and other services through a client-centered, holistic, and comprehensive approach to meeting the needs of persons through team-based care management. The program goals include prompt linkage and ongoing retention in HIV care, optimal treatment adherence, viral suppression, and reduced HIV-related mortality. This is achieved through activities such as patient navigation, directly observed therapy, health education, and coordination with the medical team. The Outreach Engagement Specialist will be a crucial member of the HIV Care Coordination team. They will perform outreach with our community partners, medical facilities and other locations the target population may frequent, and seek out individuals in need of referrals into the program to ensure timely and culturally appropriate linkages into CHASI’s program and the program’s medical partners. The Outreach Engagement Specialist will also assist with accompaniments and referral and appointment follow ups as needed.

Requirements

  • High School Diploma or equivalency required.
  • Ability to engage, communicate with, and serve the populations served by this program.
  • Conversant with HIV terminology, services, and interventions desired.
  • Lived experience with or familiarity with the target population preferred.
  • Familiarity with venues that the target population frequent and Staten Island.
  • A team player with an ability to work in diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment that motivates everyone to perform at their best.
  • Proficiency in English is required; fluency in Spanish is preferred.
  • Ability to listen to feedback and direction and take action accordingly.
  • Ability to compose client documentation, emails, and other communication in a way that is clear, accurate, and well-organized.

Responsibilities

  • Conduct street and facility-based outreach to the target populations and navigate HIV positive individuals into CHASI programs and medical care.
  • Help clients set and attend appointments for medical intervention services, plus (with signed HIPPA release) gain documentation about the outcome of such appointments; including scheduling, communicating with referral source, reminding clients of appointments, accompaniments, etc.
  • Collect and submit to CHASI’s Data Unit accurate data on persons contacted and engaged via both virtual and in-person outreach.
  • Attend one on one check in meetings with supervisor and department and other organizational meetings, as scheduled.
  • Work collaboratively with the all staff to integrate cross program activities and functions.
  • Align all activities with CHASI’s mission, core values, and culture.
  • Other duties as assigned.
