Care Coordination Associate
About The Position
The Care Coordinator is responsible for coordinating healthcare services to manage the healthcare needs of recipients. This includes logistical support, advocacy and education to assist individuals in understanding the healthcare system and activities that help recipients gain access to needed physical and behavioral health services and manage their health conditions. The incumbent provides direct assistance in gaining access to services, coordination of care within and across systems, oversight of the entire case, and linkage to appropriate services. This position directly works with interdisciplinary team to coordinate patient care within a person-centered healthcare home.
Requirements
- Bachelor’s degree and two (2) or more years experience in social sciences, mental health or related field required
- Washington State Agency Affiliated Counselor within 60 days of hire, unless one of these higher credentials are held: Counselor Agency Affiliated Registered, Counselor Agency Affiliated Certified, Mental Health Counselor Assoc (LMHCA)
- Driver License/Abstract
