About The Position

The Care Concierge is the heart of our promise to clients: that they will never have to navigate care alone. Acting as a care guide and advocate, you’ll visit clients in their homes, listen with compassion, and connect them with the right resources—whether that’s equipment, therapies, pharmacy support, or simply a helping hand to make life easier. This hybrid role is about relationships. As a Care Concierge, you’ll walk alongside clients and families, ensuring they feel supported, valued, and cared for at every step of their journey. This role will be 2-3 days a week in our Santa Fe office.

Requirements

• Valid driver’s license, vehicle, and insurance (for home visits and client support).
• Flexibility to travel locally as needed.
• Commitment to upholding Unifi Healthcare’s policies and values.

Nice To Haves

• At least 1 year of experience in customer service, healthcare coordination, or a related field.

Responsibilities

• Visit clients regularly to understand their non-clinical needs and ensure they feel heard and cared for.
• Collaborate daily with the Unifi Care Team to develop creative solutions that enhance each client’s experience.
• Help clients and families understand their benefits under EEOICPA and related programs, staying informed about policies and updates.
• Support clients with coordination of services (DME, pharmacy, transportation, home health, physical and respiratory therapy, etc.).
• Attend company and community events to strengthen connections and represent Unifi Healthcare with warmth and professionalism.
• Maintain accurate, timely records of client interactions to ensure continuity of care.
• Uphold confidentiality (HIPAA) and practice discretion in all interactions.
• Continue learning through training and education to better serve clients and families.
• Build trusted relationships with clients over time, serving as a consistent and reassuring presence.
