About The Position

This position provides a variety of patient care activities supporting the needs of the Care Coach Connect Program, including clinical duties in patients' homes and some office/clerical duties. The role involves organizing the clinical environment and assisting in patient care situations under the supervision of an Advanced Practice Clinician or medical provider. The Medical Assistant identifies patient needs, communicates data to the clinical team, and delivers quality customer service while ensuring adherence to infection control, privacy, and confidentiality policies.

Requirements

  • High school graduate or GED equivalent
  • Current, nationally recognized Medical Assistant certification or registration or must obtain the designation within 90 days of employment hire date
  • Current CPR certification or must obtain certification within 30 days of employment hire date
  • Demonstrated basic computer literacy
  • Proven knowledge of medical terminology
  • ICD-10 and CPT coding experience
  • Proven ability to react calmly and effectively in emergency situations and assume a leadership role until a higher-level clinician can arrive on the scene
  • Proven ability to travel frequently within assigned market
  • Proven ability to be flexible and adaptable to change
  • Access to reliable transportation for daily travel to various clinics as assigned
  • Valid driver’s license and current automobile insurance
  • Ability to travel, approximately 80% of the time, to meet with members in their homes, the hospital or long-term care setting, including overnight travel
  • Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease

  • 3+ years of experience as a Medical Assistant or a combination of 5+ years of non-certified medical assistant experience and certified medical assistant experience

Responsibilities

  • Performs all duties within the scope of a Certified Medical Assistant, including provider-supervised procedures, injections, diagnostic testing, and phlebotomy.
  • Coordinates healthcare screening results as directed by providers, company standards, and policies.
  • Travels with a provider to patient homes, mobile units, and other locations within the NTX Market and surrounding areas to support community-based services.
  • Performs and completes all quality program health screenings per required metrics.
  • Processes appropriate documents in an organized and accurate manner, copying physicians on patient results and quality forms.
  • Records patient care documentation in medical records accurately and in a timely manner according to company policy.
  • Reviews all medical information, test results, and survey questions prior to the patient leaving to ensure all information is captured.
  • Ensures supplies are available, stock is inventoried, log data is accurate, and supplies are rotated, following up on discrepancies.
  • Presents abnormal results to the provider promptly to address emergent conditions.
  • Prepares clinical team charts for faxing, filing, and forwarding to PCP offices post-visit.
  • Compiles results and enters chart notes prior to visits, including pulling hospital records, lab results, and specialty notes.
  • Ensures follow-up appointment dispositions are documented in CDSS and completed in eCW.
  • Reviews and ensures completion of post-visit recommendations from the provider, such as prescriptions and referrals.
  • Maintains patient confidentiality and proper release of patient information.
  • Maintains certifications (MA and CPR) and quality control standards.
  • Promotes a positive work environment.
  • May assist other team members with work coordination.
  • Performs all other related duties as assigned.
  • Operates diagnostic equipment (cannot interpret tests), assists in wound care, obtains cultures, administers non-intravenous medication, performs simple specimen collection via non-invasive techniques, collects blood specimens via venipuncture or capillary, and performs EKGs.
  • Ensures Specialist/Home Health/DME referrals are entered and followed up in a timely manner.
  • Assists in the development of Plan of Care paperwork for the Advanced Practice Clinician to review.
