Care Activator - RN/LPN
About The Position
In this role, you are the care coordinator, clinical liaison, and advocate, ensuring that every client receives the right level of support. You’ll oversee care plans, collaborate with CareHearts, and ensure services align with best practices and client needs. Whether you're an LPN or an RN, your role goes beyond clinical oversight—it’s about leadership, guidance, and ensuring care is delivered with excellence. Both LPNs and RNs will complete assessments and care plans, ensuring clients receive the appropriate level of care. As an RN, you’ll take on expanded responsibilities, including advanced clinical decision-making, case reviews, and providing higher-level oversight in care coordination within your licensed scope of practice.
Requirements
