The Care Access & Navigation Lead serves as the organizational lead and internal champion for individuals with unique abilities, with a primary focus on designing, implementing, and evolving access and navigation models that enable smooth, equitable, and coordinated care. This role functions as a centralized point of expertise and leadership, bridging patients, families, community partners, and Orlando Health practices to remove access barriers, standardize navigation workflows, and expand innovative care models across populations. The Lead drives program development, relationship-building, and continuous improvement efforts that reflect Orlando Health’s commitment to inclusive, patient-centered care.

Requirements

  • Bachelor’s degree required.
  • Valid Florida driver’s license and must be insurable under the Orlando Health insurance carrier’s requirements and standards.
  • Three (3) or more years of experience in healthcare operations, care coordination, access, navigation, or program/project leadership.

Nice To Haves

  • Experience working with individuals with unique abilities or vulnerable populations strongly preferred (e.g., Child Life, Education, Human Services, Human Resources).
  • Demonstrated ability to build workflows, influence stakeholders, and lead initiatives across teams.

Responsibilities

  • Lead the design, implementation, evaluation, and ongoing evolution of care access and navigation workflows for individuals with unique abilities.
  • Identify, pilot, and scale opportunities to expand care access models to additional populations and service lines.
  • Serve as the system subject matter expert for access and navigation needs of individuals with unique abilities.
  • Perform non-medical assessments of injury, condition, or situational barriers to guide healthcare logistics and care pathways.
  • Act as a centralized point of contact for patients, families, caregivers, providers, and operational partners to coordinate care across the continuum.
  • Collaborate with ambulatory, nursing, access, and physician leadership to align workflows, policies, and standards.
  • Develop and deliver education, training, and guidance for providers and care teams supporting individuals with unique abilities.
  • Build and maintain partnerships with community organizations, advocacy groups, and external stakeholders.
  • Proactively identify access barriers and advocate for system-level solutions that improve patient experience and outcomes.
  • Monitor, analyze, and report program outcomes, utilization, and impact to inform ongoing improvement.
  • Maintain high proficiency with organizational information systems, including ELLiE and related platforms.
  • Model exceptional customer service, professionalism, and collaboration with internal and external partners.
  • Maintain reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
  • Maintain compliance with all Orlando Health policies and procedures.
  • Regularly evaluates the effectiveness and outcomes of assigned programs and initiatives.
  • Travels to community events, partner organizations, provider locations, and other off-site settings as needed to support program development, relationship building, and care initiatives.
  • Maintains professional growth through seminars, workshops, industry publications, and continuing education related to care navigation, access, and population health.
  • Demonstrates the ability to work independently while serving as an integral leader within multidisciplinary care teams.
  • Communicates effectively across written, verbal, and digital platforms, applying strong critical thinking and problem-solving skills.
  • Documents work efforts and program outcomes in an organized, compliant, and accessible manner while adhering to confidentiality requirements.
  • Contributes to a culture of psychological safety, collaboration, and continuous improvement.
  • Strong organizational, communication, facilitation, and relationship-management skills.
