Care Access Coordinator
About The Position
The Care Access Coordinator will bridge access to care for patients by combining the roles of Hospice Intake and Supportive Care Coordinators. The coordinator will manage referrals, schedule admissions, and coordinate liaison visits, prioritizing exceptional service to referral sources. Additionally, the Coordinator will oversee Durable Medical Equipment (DME) orders and tracking to meet patient needs. For Supportive Care, the Coordinator will manage medical staff schedules, process referrals promptly, and ensure adequate support for patient visits, including coordinating Home Health Services, X-rays, and patient assessments. Your friendly, courteous demeanor is crucial in interactions with patients, families, and referral sources. Accuracy and timeliness in information provision and task completion are paramount.
Requirements
- High school diploma or equivalent is required.
- A minimum of one year of experience in hospice, home healthcare, or a customer service-related position is preferred.
- Demonstrate exceptional customer service.
- Exhibited understanding of and acceptance of the hospice philosophy.
- Demonstrated ability to work effectively both independently and as part of a team.
- Proficiency in multitasking in a busy environment, managing multiple assignments, and meeting deadlines with attention to detail, efficiency, and accuracy.
- Ability to communicate effectively with medical professionals across various disciplines.
- Basic computer skills are required, including proficiency in Microsoft Word and Outlook.
- Demonstrated ability to efficiently multitask in a busy and stressful environment with a focus on detail and accuracy.
- Strong customer service skills are essential.
- Proficient in computer applications such as Microsoft Word, Electronic Medical Records programs, Outlook, Excel, and scheduling software.
- Possesses effective problem-solving skills with the ability to prioritize and implement solutions.
- Demonstrated commitment to providing quality, timely, and cost-effective patient care.
Nice To Haves
- an associate degree in a related field is preferred, although candidates with commensurate experience will be considered.
Responsibilities
- Respond to referral calls, providing appropriate information and completing associated paperwork and records.
- Explains hospice services to callers and visitors, as needed.
- Orders DME, tracks and documents appropriately.
- Communicates, as needed, with DME company, attending physician, and/or care facilities personnel to track admission order, certification, or certification of terminal illness.
- For Medicare patients, run financial verification of eligibility dates and previous hospice benefits.
- Enters referral information in electronic medical record software.
- Works with finance department to facilitate proper billing of hospice services.
- Schedules admissions visits.
