Care Access Coordinator- Cayuga Park Cayuga Cancer Center (CHOA)- Full Time- Days
About The Position
Centralus Health, an affiliation of Arnot Health and Cayuga Health, is seeking a Care Access Coordinator to join the supportive service team at the Cancer Resource Center of the Finger Lakes (CRCFL). This role will assist individuals impacted by cancer by addressing financial needs, identifying alternative payment sources, and navigating Medicaid benefits and other resources to reduce barriers to care. The Care Access Coordinator will collaborate closely with both Cayuga Health System and CRCFL to ensure patients receive holistic support, access to critical resources, and assistance throughout their cancer journey.
Requirements
- High School diploma or equivalent experience.
- Proficient in Microsoft Office suite.
- Strong interpersonal skills with experience in customer service.
- Good time management and prioritization skills.
Nice To Haves
- Experience working in a healthcare or community resource setting.
- Familiarity with Medicaid and financial assistance programs.
- Background in patient advocacy, social work, or case management.
Responsibilities
- Coordinate benefit investigations and assist clients with applying for grants and alternative payment sources.
- Ensure clients are connected to resources that address their financial needs.
- Screen clients for Medicaid eligibility and assist them in completing and submitting applications.
- Track the progress of applications and provide updates to clients on their status.
- Provide emotional support to clients and answer inquiries related to financial assistance and program eligibility.
- Direct clients to appropriate services as needed, offering both information and support.
- Maintain accurate client records in databases.
- Generate reports on client progress, financial assistance efforts, and other administrative tasks.
- Support general office coordination.
- Work as part of the supportive service team at the Cancer Resource Center of the Finger Lakes, actively engaging with clients to ensure they receive comprehensive assistance.
- Collaborate with CRCFL staff to connect patients with peer support programs, educational workshops, and community events.
- Assist with CRCFL strategic priorities, events, and special projects, ensuring patients benefit from the organization's services and advocacy efforts.
- Contribute to the success of team initiatives and community outreach efforts.
Benefits
- Standard rate of $24.00 - $27.00 per hour plus benefits
