About The Position

We are seeking a full-time team member to join our Cardiopulmonary Department. This position works Four 10-hour shifts each week (Monday, Wednesday, Thursday, and Friday) from 7:00 a.m. to 5:00 p.m. Provides direct and/or indirect care for individual patients. Communicates with and on behalf of the patients and coordinates patient care with other health team members. Organizes those aspects of care for which they are accountable. This individual will be primarily responsible for cardiac rehab. Other duties as assigned.

Responsibilities

  • Assesses each patient on admission to rehab program and determines appropriate care plan
  • Develops, implements, and keeps current on all individualized treatment plans.
  • Participates in the medical plan of care to promote an integrated health care plan
  • Evaluates patient responses to exercise and documents appropriately
  • Treats patient in the ER, IPC, and OP setting. Tasks include but are not limited to: EKGs, breathing treatments, chest physiotherapy, patient education, heart monitor application and download, pulmonary function testing.
  • Provides patient education as indicated related to illness, medication management, symptom managements, management of the disease process, admission and discharge instructions, risk factors, exercise instructions and precautions.
  • Prioritizes, plans, and organizes the delivery of care to patients in order to use time and resources effectively and efficiently.
  • Delegates aspects of care to coworkers both in and outside of the Cardiopulmonary department consistent with their levels of education and expertise in order to meet patient needs.
  • Maintains accountability for own care and care delegated to others to assure adherence to ethical and legal standards.
  • Maintains patient confidentiality and patient rights.
  • Recognizes changes in patient's conditions and notifies physician per policy standards.
  • Plans, promotes, and participates in patient education and rehabilitation.
  • Administers medications and treatments to patients per physician orders.
  • Participates in educational activities, quality improvements activities, standards development and review, hospital committees and other related activities.
  • Complies with hospital and cardiopulmonary policies.
  • Assists in the orientation of new staff.
  • Demonstrates willingness to support staffing and scheduling needs.
  • Performs functions other than described above due to extenuating circumstances.
  • Traumas and Codes: Reports to primary ED nurse PRN.
