Our client is leading Indiana’s 5-year Rural Health Transformation Program, including the Make Rural Indiana Healthy Again Regional Grants, a historic $600M investment to strengthen healthcare access, prevention, and system resilience across 64 fully rural and nine partially rural counties. This initiative brings together state leadership, healthcare providers, and community organizations to design and implement solutions that reflect local needs while advancing a shared, statewide vision for better health outcomes. This role leads the design and statewide implementation of cardiometabolic health strategies that directly improve prevention, early detection, and long-term outcomes in rural communities. About the Role The Cardiometabolic Health Program Lead serves as the statewide leader responsible for coordinating and advancing Initiative 5 of the Rural Health Transformation Program (RHTP). This initiative focuses on improving cardiometabolic health outcomes through prevention, early detection, lifestyle interventions, and stronger integration between clinical care and community-based services. This role blends strategic leadership with hands-on implementation support. You will design program models, guide regional execution, and ensure alignment with Indiana Department of Health priorities and CMS expectations. Working closely with IDOH leadership, Regional Grant Managers, and statewide partners, you will help ensure consistent, high-quality implementation across all rural regions. As a visible leader within the initiative, you will connect policy, program design, and real-world delivery, ensuring that evidence-based strategies translate into measurable improvements in population health.
