About The Position

As the Pharmacy Program Manager, you will support Growing Access to Hospital Post-Discharge Medications by helping rural hospitals implement and strengthen Meds to Beds programs across Indiana. In this role, you will work closely with state agencies, rural hospitals, and other stakeholders to support hospital-based medication access programs that ensure patients receive prescribed medications before discharge. You will help drive implementation, build partnerships, provide technical assistance, and support performance tracking to improve care transitions and reduce access and transportation barriers. This is a highly collaborative role that requires strong relationship-building, program coordination, and the ability to support hospitals and partners through implementation, problem-solving, and continuous improvement.

Requirements

  • A medical or pharmacy-related degree, or a degree in public health, health science, health policy or administration, social science, or other relevant field
  • Ability to plan, prioritize, and manage multiple projects simultaneously
  • Ability to synthesize analytical insights into clear, objective, and actionable recommendations
  • Strong written and verbal communication skills, with the ability to present to diverse audiences
  • Ability to build and maintain effective relationships with agency staff, hospital leaders, and stakeholders
  • Proficiency with Microsoft Office tools, including Word, Excel, PowerPoint, and Publisher
  • Detail-oriented and self-motivated approach to managing work and priorities

Nice To Haves

  • Experience in rural health initiatives
  • Clinical background with experience in pharmacy, hospital, primary care, ambulatory, or community health settings
  • Knowledge of federal and state policies and regulations governing pharmacy practices
  • Experience in leadership, facilitation, adult education, and project management
  • Familiarity with Pharmacy Benefits Management (PBM), Pharmacy Services Administration Organizations (PSAOs), and point-of-sale pharmacy systems

Responsibilities

  • Lead coordination efforts that help rural hospitals launch and sustain Meds to Beds programs.
  • Coordinate and facilitate Meds to Beds programs with rural hospitals and key stakeholders across Indiana
  • Support participating hospitals in implementing program models that improve access to post-discharge medications
  • Ensure hospitals receiving grant funds are meeting deliverables outlined in work plans and complying with contract requirements
  • Identify implementation barriers and support problem-solving to keep programs moving forward
  • Build strong partnerships across hospitals, agencies, and external organizations to drive adoption and long-term success.
  • Foster partnerships with rural hospital administrators to build hospital buy-in and overcome program implementation barriers
  • Work closely with hospital CEOs, pharmacy leadership, and other internal stakeholders to support implementation
  • Collaborate with external partners, including the Indiana Pharmacy Association and other stakeholders, to strengthen program delivery
  • Share knowledge, best practices, and successful models from existing Meds to Beds programs with participating hospitals and health systems
  • Serve as a key liaison connecting hospitals with the resources, vendors, and support needed for successful implementation.
  • Provide on-site technical assistance to participating hospitals and serve as a liaison between hospitals, technical support resources, and IDOH leadership
  • Develop relationships with vendors, including medical records systems, couriers, pharmacies, Pharmacy Benefits Managers, Pharmacy Services Administration Organizations, and other potential contractors or sub-awardees
  • Help hospitals navigate operational and resource needs related to program development and implementation
  • Support coordination between hospitals and external partners to ensure timely, effective program execution
  • Support data collection, outcome measurement, and reporting to demonstrate impact and guide continuous improvement.
  • Develop reporting guidelines and systems to assess program outcomes
  • Provide technical support to hospital staff responsible for tracking data and project outcomes
  • Work with IDOH leadership to coordinate measurement, analysis, and reporting of progress on defined KPOs
  • Provide regular updates and feedback to IDOH leadership on progress, performance, and opportunities for improvement
