About The Position

UVA Cardiology at Culpeper Medical Center is seeking a full-time Advanced Practice Provider (APP) to support its growing cardiology program. This role offers an outpatient clinical role based in UVA Cardiology Culpeper, allowing the APP to practice at the top of their license within a collaborative team. The APP will work closely with three cardiologists and one APP, supported by experienced nursing staff, medical assistants, and clinic leadership. The role involves partnering with pharmacists to optimize medication therapy and patient adherence, working with social workers to address behavioral health and social needs, and collaborating with registered nurses and navigators for care coordination and follow-up. Additionally, the APP will coordinate care with imaging, heart failure, and electrophysiology subspecialists, participate in interdisciplinary case conferences and care planning meetings, and collaborate closely with physicians, hospital administrators, and other healthcare professionals to improve workflow and patient outcomes. Requirements

  • Nurse Practitioner or Physician Assistant

Responsibilities

  • General cardiology outpatient clinic visits
  • Inpatient cardiology consults and rounding in collaboration with physicians
  • Care coordination across inpatient and outpatient settings
  • Evaluate, diagnose, and manage new and follow-up patients with General Cardiology and complex cardiovascular prevention needs.
  • Perform cardiovascular assessments; order and interpret diagnostic testing; develop and implement individualized treatment plans.
  • Manage cardiovascular risk factors and chronic conditions
  • Manage general cardiovascular conditions such as hypertension, coronary artery disease, heart failure, arrhythmias, and valvular disorders.
  • Provide patient and family education regarding disease processes, treatment options, procedural care, and long-term self-management.
  • Coordinate diagnostic testing, referrals, and follow-up care.
  • Document clinical encounters accurately and timely in the electronic medical record (EPIC).
