About The Position

Cardiology Services (Full-time, Varied Shift) Job Summary: The Cardiac Navigator Nurse Practitioner (NP)-Cardiac Services practices in collaboration with the supervising physician and performs overlapping medical and surgical procedures for specialty population of patients with critical acuity and participates in service coverage needs 24 hours per day 7 days per week. Serves as a resource for multidisciplinary healthcare team members, patients, and families. Utilizes expertise in the delivery of whole-person care. Practices within the NP scope of practice pursuant to standardized procedures and follows policies, protocols, techniques, guidelines, standardized procedures, and state and federal regulations. Functions as an educator, consultant, clinical practitioner, researcher, and leader. Performs direct application of clinical expertise to patients and families and patient care staff. Plans, implements, coordinates and evaluates services for the Cardiothoracic and Interventional Cardiology and Endovascular patients. Performs daily patient rounding-coordination of care and communicating plan of care to the patient and family and patient care team. Optimizes post hospital care coordination including Cardiac Rehab referrals. Serves as a liaison for critical inter-hospital Cardiothoracic and Interventional Cardiology and Endovascular transfers in collaboration with nursing services. Serves as a liaison for the ST-Evaluation Myocardial Infarction (STEMI) and Society of Chest Pain Center accreditation and data collection and validation. Performs other duties as needed.

Requirements

  • Master's Degree and completion of Nurse Practitioner curriculum required.
  • Minimum two years of clinical nursing experience in area of specialty required.
  • Good physical and emotional stamina.
  • Demonstrates an understanding of normal growth and development of the pediatric, adolescent and adult population and care of these patients as appropriate to unit assigned, as well as the elderly and geriatric patient population and the care of these patients as appropriate to unit assigned.
  • Able to read; write legibly; speak in English with professional quality; troubleshoot and calibrate patient care equipment.
  • Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction.
  • Able to think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail.
  • Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place and patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.
  • Active California Registered Nurse (RN) license required.
  • California Nurse Practitioner (NP) Certification required.
  • Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications from the American Heart Association required.

Nice To Haves

  • Minimum one year Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP) experience preferred.
