About The Position

The Navigator supports Community Progress Council’s outreach and engagement efforts across York County by recruiting and enrolling new participants, re-engaging former participants, and representing CPC in the community. Serving as the first point of contact, the Navigator provides welcoming, high-quality customer service to individuals and families by phone and in person. With strong knowledge of CPC programs, this role builds effective relationships with families and community partners and coordinates integrated services that support progress toward self-sufficiency. The Navigator provides culturally responsive, family-centered support, responds to individuals in crisis by addressing immediate needs, and ensures timely transitions to the appropriate program coach for ongoing support.

Requirements

  • GED/HS diploma is required, college experience preferred. Relevant education and professional experience in social services, communications, human development, public health, or a related field supporting individuals, families, or communities.
  • Strong verbal and written communication skills with the ability to engage effectively with diverse populations; bilingual proficiency in English and Spanish and/or Haitian Creole preferred.
  • Ability to work collaboratively with team members, community partners, and clients while adapting to changing priorities and responsibilities.
  • Reliable transportation and a valid Pennsylvania driver’s license required; dependable attendance and timely responsiveness via phone, email, teleconferencing, and work-issued mobile devices.
  • Willingness to work across multiple CPC locations, including occasional evenings and weekends, to meet client and program needs.
  • Proficient in Microsoft Office Suite within a Windows environment.

Responsibilities

  • Maintain a comprehensive understanding of CPC programs and eligibility requirements and actively recruit and enroll families across all CPC programs.
  • Manage walk-in clients, phone inquiries, and scheduled appointments efficiently while collaborating with team members to ensure seamless service delivery.
  • Develop and maintain strong partnerships with community agencies, health organizations, and local groups to coordinate referrals, connect families to services, and help identify gaps in resources.
  • Conduct client interviews and complete holistic assessments to determine eligibility, program placement, and appropriate referrals; partner with clients to problem-solve and set goals.
  • Ensure timely and accurate data entry across multiple systems of record, maintain complete client documentation, and uphold all confidentiality requirements.
  • Advocate on behalf of clients to support timely, effective, and appropriate access to services.
  • Assist clients with applications and documentation related to self-sufficiency, including employment, utilities, childcare, and other basic needs.
  • Prepare timely, accurate reports to meet internal, external, and funder requirements.
  • Represents Community Progress Council in the community by participating in committees, health fairs, outreach events, and community-based activities.
  • Participate in ongoing professional development to enhance skills and remain current with best practices.
  • Perform administrative support tasks, including sorting mail and faxing, copying, and scanning documents.
  • Perform other duties as assigned to support program and organizational goals.
