About The Position

Sentara Norfolk General's Heart Hospital in Norfolk, Virginia is seeking a Integrated Care Manager, RN Cardiac Surgery Navigator for the Cardiac Surgical - Procedural IMCU! The Integrated Care Manager, RN Cardiac Surgery Navigator is responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Functions in one of the following practice settings: Acute Care, Service Lines, Ambulatory/Community-based, Home Health, and Long Term Care. Cardiac Procedural/Surgical IMCU Overview: This 24-bed cardiac surgical/procedural IMCU provides care to pre and post cardiac surgery patients to include: bypass surgery, cardiac valve repairs/replacements, thoracic surgery/lung diagnostics, and other cardiac based procedures and treatments such as ablations, left atrial appendages, ICDs, and permanent pacemakers. • Provides a high level of care to patients with cardiac arrhythmias, heart failure, and chest pain. • Nurses handle 3-4 patients on this unit based on acuity level.

Requirements

  • Associate’s Degree or Diploma in Registered Nursing (required)
  • Virginia RN License Virginia or Compact/Multi-State Eligible.
  • All Direct Care RN's required to have BLS within 90 days of hire.
  • Critical Care/IMCU ACLS within 1 year of hire
  • 3 years of nursing experience

Nice To Haves

  • Bachelor of Science Nursing- BSN or MSN
  • 3 years of Case Management
