About The Position

Sentara Service Line Administration is seeking a full-time Cardiac Care Continuum Navigator in Virginia Beach, VA. This is a hybrid position requiring three days per week on-site and two days remotely. The SR. Navigator is responsible for maintaining and reviewing quality metrics and fallouts, program development, participating in system initiatives, data analysis, maintaining programs, and performing duties appropriate to the assigned service line. They also onboard new team members and provide guidance and mentorship to other navigators. Daily activities, accounting for 60% of the time, include daily rounds, discharge planning, physician relations, pre-operative chart/pre-admission testing process, and monitoring the destination center surgery schedule. Program development activities, accounting for 40% of the time, include patient education, program development, patient satisfaction, continuous improvement, data collection, and marketing, ensuring the long-term success and growth of the program.

Requirements

  • Bachelors Level Degree in Nursing, Healthcare Administration, Public Health, Business Administration, Clinical Healthcare, or related field (Required). 5 - 10 years of relevant work experience in lieu of education, however candidate must obtain BS degree within 18 months of hire.
  • For Registered Nurse candidates, BSN (Required).
  • Certified Occupational Therapy Assistant (COTA) - Certifications - National Board Certification In Occupational Therapy (NBCOT)
  • Licensed Nurse Practitioner (NP) - Certification - Other/National
  • Licensed Nurse Practitioner (NP) - Nursing License - Virginia Department of Health Professionals (VADHP)
  • Occupational Therapist License (OT) - State license - Virginia Department of Health Professionals (VADHP)
  • Physical Therapist Assistant License (PTA) - State license - Virginia Department of Health Professionals (VADHP)
  • Physical Therapist License (PT) - State license - Virginia Department of Health Professionals (VADHP)
  • Physician Assistant License (PA) - State License - Other/National
