About The Position

The Child Abuse Prevention Intervention Treatment (CAPIT) Care Coordinator provides case management, parenting education, linkages, referrals, and other ongoing support to CAPIT clients. The CAPIT Care Coordinator works collaboratively with the program staff to assess, support, and link clients to needed community services. The CAPIT Care Coordinator documents all services according to the Department of Children and Family Services (DCFS) standards and Penny Lane Centers documentation guidelines. The CAPIT Care Coordinator demonstrates strong verbal and written communication skills with clients, community members, outside agencies, and colleagues. The Care Coordinator maintains a calm, ethical, and professional demeanor in clients’ homes and schools. Effective teamwork, collaboration, and care coordination with Penny Lane Centers staff, DCFS, DMH, and other partners are essential. This role requires flexibility, strong time management, the ability to work independently, and excellent customer care skills. The CAPIT Care Coordinator follows all Penny Lane Centers policies and procedures regarding the Heart Centered Culture, cultural sensitivity, and cultural awareness. The CAPIT Care Coordinator supports the agency’s mission and vision in their interaction with children, youth, families, and partner agencies.

Requirements

  • BA or BS in Counseling or Psychology plus four years’ experience in the provision of services to low income and ethnically diverse clients; OR MSW, MFT, MS, or MA in counseling or psychology, or be a certified drug counselor, plus four years’ experience in the provision of services to low income and ethnically diverse clients.
  • Bilingual Spanish required (verbal and written skillset).
  • Valid California driver’s license.
  • Current automobile insurance.
  • Maintain a clean driving record acceptable to the organization’s insurance company.
  • Have immediate access to his or her vehicle during work hours.
