Requirements

  • High School diploma or equivalent
  • Registered LPN with active unrestricted license in the State of Nevada
  • 1+ years of experience in home care, community health or medical/surgical patient care setting providing direct patient care
  • High knowledge of customer service principles and ability to remain calm and composed in pressured situations
  • PC skills with Windows, Outlook or other email systems, Electronic Medical Records systems and experience working with a clinical application system

Responsibilities

  • Ensuring timely and accurate processing of home care referrals.
  • Obtaining initial completed referral and general patient information to coordinate the state of care.
  • Coordinating with the patient and/or family/caregiver regarding home care services including collection of demographic data and scheduling preferences.
  • Providing authorizations for home care services, as applicable.
  • Ensuring that scheduling and data entry requirements are consistently accomplished to meet patient needs and state and federal regulations.
  • Assisting with the general office functions for the agency to include coordinating services and answering patient calls and other duties as assigned.
  • Accepting calls from referral sources and patients/families/caregivers and obtaining accurate and up-to-date information related to the admission for home care services.
  • Providing expert customer service in interactions with referral sources and clients.
  • Obtaining required patient information to initiate admission for home care services including, but not limited to, physician orders for home care services, referral information, hospital discharge summary (as applicable) and other documentation.
  • Assisting with telephone coverage for the department and agency and coordination of patient care issues.
  • Assuming individual responsibility for professional competence and professional nursing practice.
  • Assisting with initial patient visit scheduling if applicable and coordinating services to other disciplines and departments such as Southwest Medical Pharmacy services.
  • Participating in rotation of office coverage for weekend and holidays.
  • Promoting positive customer service at all times both internally and externally in interactions with patients, families, referral sources, vendors, co-workers of the department and agency.
  • Assisting with new employee orientation.
  • Attending mandatory meetings and in-services.
