About The Position

Yolo County Children's Alliance (YCCA) is seeking a collaborative, solutions-focused CalAIM Program Manager to help implement and manage our growing CalAIM and community health initiatives. This is a hands-on leadership role for someone who can take complex healthcare and program requirements and turn them into practical workflows, strong teams, and effective services. You'll support CalAIM, Enhanced Care Management (ECM), Community Health Worker (CHW), and other Medi-Cal-funded programs while helping ensure services are high-quality, compliant, and responsive to community needs.

Requirements

  • Strong project and program management skills.
  • Experience supervising or leading staff.
  • Excellent organizational, communication, and problem-solving skills.
  • Experience managing workflows, deadlines, documentation, and compliance requirements.
  • The ability to use data to monitor performance and improve programs.
  • Experience working with healthcare, human services, community health, social services, or other publicly funded programs.
  • A commitment to culturally responsive and person-centered services.
  • A valid California driver's license, auto insurance, and reliable transportation are required.
  • This position requires regular local travel and periodic evening and weekend availability.
  • A fingerprint and background check is required.

Nice To Haves

  • Experience with Medi-Cal, CalAIM, ECM, Community Supports, CHW services, managed care, healthcare operations, or community-based organizations is strongly preferred.

Responsibilities

  • Manage program implementation plans, timelines, deliverables, and day-to-day operations.
  • Translate CalAIM, Medi-Cal, health-plan, and contractual requirements into practical workflows and procedures.
  • Supervise, coach, and support assigned program staff.
  • Oversee referrals, enrollment, caseloads, service delivery, documentation, and follow-up.
  • Monitor program performance, quality, compliance, and outcomes.
  • Support billing readiness and ensure documentation meets program requirements.
  • Coordinate with healthcare providers, health plans, government agencies, consultants, and community partners.
  • Prepare reports and identify operational challenges, risks, and opportunities for improvement.
  • Assist with audits, monitoring, site reviews, and corrective actions.
  • Build sustainable systems and processes that support long-term program success.
  • Represent YCCA at partner and community meetings and events.
