Expand lines of service as defined by management by developing and retaining referrals from physicians and other key non-physician referral sources. This role involves collaborating closely with hospital discharge planners, social workers, case managers, physicians, and management, as well as providing education staff to generate referral sources. The representative will develop strong relationships with key referral sources, identify opportunities for partnering and growth, and manage leads from referral sources to ensure an efficient admissions process for clients. The goal is to increase the number of referral sources within a facility, coordinate with the clinical assessment team on referrals, and educate patients, families, the community, and other referral sources on programs, services, and amenities provided by the facility. This role requires frequent travel within the community, scheduling meetings, and developing contacts. The representative will document referral sources in a contact management system, develop events and educational forums, maintain existing referral relationships, and utilize analytic tools like Salesforce to plan and evaluate activity. Representation of the facility in professional forums and communication of its mission in all outreach interactions are key. The role also involves creating outreach initiatives to ensure regular communication with referral sources through various professional activities and providing necessary information to facilitate admissions.
