The purpose of this role is to expand lines of service as defined by management by developing and retaining referrals from physicians and other key non-physician referral sources. This involves collaborating closely with hospital discharge planners, social workers, case managers, physicians, management, and education staff to generate referral sources. The representative will develop strong relationships with key referral sources, identify opportunities for partnering and growth, and manage leads to ensure an efficient admissions process. The role aims to increase the number of referral sources within a facility and educate patients, families, the community, and other referral sources on the facility's programs, services, and amenities. Travel within the community is frequent, involving scheduling meetings and developing contacts. The representative will document referral sources in a contact management system, develop events and educational forums, maintain existing referral relationships, and utilize analytic tools like Salesforce to plan and evaluate activity. They will represent the facility in professional forums, communicate the facility's mission in all outreach interactions, and create outreach initiatives to ensure regular communication with referral sources through various channels. The role also involves providing necessary information to facilitate admissions and performing other assigned functions.
