Requirements

• Associate degree in one of the helping professions, such a social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice, from an accredited college or university and will have a minimum of two years of experience in a community-based behavioral healthcare setting.
• Bachelor's degree from an accredited college/university in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.
• Master's degree from an accredited college/university in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice.
• Individuals with a Bachelor’s degree only in a field other than a helping profession will have a minimum of two years of professional experience in a community behavioral health care setting.
• Certification as a Psychiatric Rehabilitation Practitioner may be substituted for one year of experience.
• Valid driver’s license required- no more than one moving violation within the past 12 months

Responsibilities

• Create and Maintain: A wellness and recovery-oriented environment by using person-first language, respectful documentation, person-centered planning, and evidence-based practices
• Create and Maintain: A culture of compliance with internal and external policies, regulations, laws and high ethical and professional standards.
• Community Support Services Responsibilities: Contributes to the Comprehensive Rehabilitation Needs Assessment on an ongoing basis.
• Community Support Services Responsibilities: Participates with the licensed staff in the development, implementation, monitoring and updating of the individualized rehabilitation plan/Individual Recovery Plan (IRP).
• Community Support Services Responsibilities: Perform therapeutic rehabilitative skill development as outlined in the CSS Regulations.
• Community Support Services Responsibilities: Offer services appropriate to meet person’s served needs as identified in the Comprehensive Rehabilitation Needs Assessment (CRNA) and included in each individuals IRP.
• Community Support Services Responsibilities: Perform crisis intervention as indicated
• Community Support Services Responsibilities: Coordinate and manage services in conjunction with the team.
• Community Support Services Responsibilities: Supportive counseling, medication education, mental health education and early crisis intervention
• Community Support Services Responsibilities: Engages people receiving services in the treatment process.
• Community Support Services Responsibilities: Utilizes Illness Management and Recovery, WRAP and other relevant health and wellness curriculum to ensure overall wellness is a key focus in the individual’s recovery plan
• Community Support Services Responsibilities: Perform evidence-based practices that are appropriate for implementing community support services. This may include Motivational Interviewing, Cognitive Behavioral and Educational Strategies, Trauma Informed Care, Career Services, Permanent Supportive Housing, IMR and WRAP.
• Community Support Services Responsibilities: Mental health education, community living skill training, and supportive counseling in the home and other natural environments to person served and significant others
• Community Support Services Responsibilities: 24-hour on-call coverage on a rotating schedule
• Community Support Services Responsibilities: Technical assistance and education to members of the team in the area of recovery and wellness. Works with the team to develop materials and strategies for teaching wellness strategies including Wellness Recovery Action Plans, instilling hope, promoting choice and empowerment, and spirituality, in conjunction with the team
• Community Support Services Responsibilities: Serves as a resource for all team members to ensure health literacy principles are translated into services and supports offered for all persons served, especially those who are at risk for metabolic syndrome
• Community Support Services Responsibilities: Consults with other service providers and resource people regarding the assessment, treatment and rehabilitation needs of the people receiving services
• Community Support Services Responsibilities: Completes chart documentation for assigned individuals according to CSS regulations
