About The Position

This position supports the BIAC mission by providing a person-centered approach and trauma-informed care to individuals with brain injuries or their caregivers. As a Brain Injury Resource Facilitator, work with clients across the state of all ages by phone, in-person (as needed), or email to provide guidance, support, and hope while navigating resources.

Requirements

  • Bachelor’s degree, or equivalent experience, in areas related to the position.
  • Experience navigating systems in a human services, case management, social work, or other applicable setting.
  • Knowledge of community, state, and federal resources for brain injury, disability, or human services specific to the region this position serves.
  • Access to reliable transportation for travel within the region as needed.
  • Can read and interpret documents.
  • Can draft routine reports and correspondence.
  • Can speak effectively before groups.

Nice To Haves

  • Experience working with people with cognitive disabilities and/or behavioral health challenges.
  • Training in harm reduction, disability rights, and trauma informed care
  • Bilingual (English and Spanish)

Responsibilities

  • Work with clients statewide to help brain injury survivors and their support networks access and understand the benefits and resources available in their community.
  • When necessary, collaborate with other Resource Facilitators to schedule face-to-face meetings to meet client’s needs.
  • Create new or update existing client files within Salesforce including at minimum, baseline demographics, injury, and contact information.
  • Open client cases with achievable goals to address client needs, documenting summary of activity within 48 hours of interaction.
  • Support brain injury survivors in troubleshooting challenges, even in cases where they may be uncertain about the resources they require.
  • Refer clients to internal programs and/or community agencies to meet resource needs.
  • Work collaboratively with team members when they encounter challenges in finding resources for clients or determining the appropriate course of action
  • Answer calls and respond to voicemails on the Brain Injury Association of Colorado’s main line number.
  • Establish connections with external service providers to maintain an up-to-date inventory of resources and engage/participate in meet and greet opportunities.
  • Take part in staff training sessions and meetings to collaborate with the team and enhance professional skills.
  • Provide education to clients, support networks, and professionals regarding the fundamentals of brain injuries and the significance of addressing symptoms, including the provision of educational materials as necessary.
  • Performs other related duties as assigned.
