Requirements

  • Must be able to connect patients to Epic (EMR).
  • Must be able to educate patients on self-monitoring blood pressure (SMBP) devices and how to take their pressure.
  • Must be able to document encounters in the patient’s medical record.
  • Must be able to make outreach calls to schedule patients for appointments.
  • Must be able to communicate with care team members (MA, Nurses, PCP) to facilitate client care.
  • Must be able to monitor referral reports and outreach to patients not scheduled for visits and/or missed appointments.
  • Must be able to assist PHM in device inventory and tracking of devices handed out.
  • Must be able to assist PHM in reviewing and analyzing data on patients with SMBP for BP monitoring, f/u appointments, patient demographics.
  • Must complete a background check before beginning employment.

Responsibilities

  • Educating patients on hypertension, how and when to take their blood pressure, and help to connect these to Epic (EMR).
  • Working collaboratively with project leads to schedule patients and manage blood pressure clinic.
  • Educating patient/patient’s family on self-monitoring blood pressure (SMBP) devices and how to take their pressure.
  • Documentation of these encounters will be in the patient’s medical record.
  • Making outreach calls to schedule patients for appointments.
  • Communicating with care team members (MA, Nurses, PCP) to facilitate client care.
