We are seeking a Blended Case Manager to join our team around our Monaca, PA location. Earn $22.67/hour with the selection of the Enhanced Pay Option. Under the direction of the Blended Case Management Supervisor, you will: Serve as the primary contact for up to 30 individuals in the community Develop, coordinate, and implement personalized service plans Provide case management, service planning, and coordination with other agency providers Offer supportive counseling to help individuals achieve their goals Work alongside county reentry grants to support individuals transitioning from jail back into the community Participate in on-call responsibilities as needed Merakey is a non-profit provider of developmental, behavioral health, and education services. Requirements

High school diploma or GED AND AT LEAST 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required.
2+ years’ experience in public or private human service, with 1 year being direct care required. Experience must be with clients over the age of 18.
Valid Driver's license required.
All clearances required by OMHSAS regulations.
