Under the supervision of the Director Community Operations, the Blended Case Manager (BCM) Supervisor is responsible for the direct supervision of up to nine (9) targeted case managers who provide services to individuals with severe and persistent mental illness, including both adults and children/adolescents. The supervisor will be available to consumers 24 hours per day, 7 days per week, utilizing a weekly on-call rotation. This role supports case managers in helping clients access necessary community resources and achieve their recovery goals. The BCM Supervisor also participates in assessing program needs and ensuring high-quality service delivery. ESSENTIAL FUNCTIONS: The essential functions of this position consist of the ability to perform basic math functions, write, read, and comprehend what is read. Expressive and receptive language skills are also required in the execution of the responsibilities below. Twisting, bending, and other physical movements may be required at varying degrees. Use of telecommunications equipment is required along with the data input and gathering via a computer. All the above may be performed with or without reasonable accommodation. This position requires the ability to move and work within multiple residential and office settings and requires frequent travel to and from client’s residences in order to provide services.
