About The Position

Nadia Care is dedicated to improving pregnancy, birth, and postpartum journeys for women and their families by transforming maternity care delivery. The organization focuses on building trust-based engagement and providing wrap-around support to expectant mothers through home visits, in-person interactions, and virtual engagement. Nadia Care assists members with care navigation, doula support, lactation, nutrition, housing, transportation, and other needs, leveraging technology for virtual support. They act as a trusted source for support, resources, and information throughout the pregnancy, delivery, and postpartum periods, complementing, not replacing, primary obstetric care by addressing health-related social needs not typically covered by doctors' offices. Requirements

  • 3-5+ years in patient onboarding, member engagement, customer service, community health, or healthcare navigation.
  • Fully fluent in English and Spanish (verbal and written) with strong conversational fluency and comfortable using non-technical terminology with members.
  • High emotional intelligence, active listening skills, and the ability to build immediate rapport while guiding members through structured talk tracks.
  • Comfortable navigating CRM platforms, virtual communication tools, and scheduling software (e.g., Acuity, Spruce, EHRs, MacOS).
  • Comfortable working independently in a fast-paced, high-growth startup environment while hitting daily outreach quality and engagement targets.
  • High school diploma or GED equivalent.
  • Reliable home internet access and a quiet space for taking calls.
  • Experience with Medicaid populations is a strong plus.

Responsibilities

  • Serve as the vital first point of contact for new members joining Nadia Care.
  • Conduct up to 7 comprehensive, virtual Welcome Visits per day.
  • Assess members' baseline needs during the first visit.
  • Educate members on Nadia Care's services.
  • Identify and escalate any urgent medical or social needs to the clinical team.
  • Establish a warm, trusting relationship with members.
  • Coordinate directly with Maternity Navigators and clinical specialists to ensure smooth care transitions.
  • Pass critical intake notes to the core team.
  • Maintain rigorous data accuracy across platforms by logging interactions, scheduling visits, and tracking onboarding progress in systems like Athena.
